Fleksibl sistoskopi yapılanlarda lidokain jel ve lidokain jel+intramuskuler analjeziğin lokal anestezik etkinliği açısından karşılaştırılması

AMAÇ: Bu çalışmada fleksibl sistoskopi uygulanan erkek hastalarda %2 lidokainli jel ve %2 lidokainli jel -diklofenak kombinasyonunun ağrı üzerine etkilerini araştırılması amaçlandı.
Gereç ve YÖNTEMLER: 04/2016 ve 04/2017 tarihleri arasında fleksibl sistoskopi uygulanan erkek hastaların kayıtları retrospektif olarak incelendi. Sistoskopi nedenleri alt üriner sistem semptomları nedeniyle tanısal yada mesane tümörü nedeniyle kontrol amaçlıydı. Üretra darlığı olan, sistoskopi esnasında ilave işlem yapılan (biyopsi, kateter çekme vs. ) ve duyusal nörolojik defisiti olan hastalar çalışmaya dahil edilmedi. Hastalar kullanılan analjezi türüne göre 2 gruba ayrıldı. Grup I’deki 100 hastaya %2 lidokainli jel, grup II’deki 109 hastaya jele ilaveten intramuskuler diklofenak uygulandı. Jel grubunda işlemden 15 dakika önce ve hemen işlem öncesi üretraya 10 cc %2 lidokainli jel instile edildi. Diğer gruba ise aynı jel instilasyonuna ilaveten yarım saat önce intramuskuler 75 mg diklofenak uygulandı. Tüm işlemler aynı ürolog tarafından gerçekleştirildi. İşlem sonrası hastalara VAS skorları dolduruldu ve gruplar karşılaştırıldı.
BULGULAR: Grup I’deki hastaların ortalama 63±14,8 yıl iken Grup II’de 61±15,0 yıldı (p=0,42). Ortalama işlem süresi Grup I’de 376±28 ve Grup II’de 9 saniyeydi (p=0,071). Gruplar arasında ağrı skorları arasında istatistiksel olarak fark saptandı. (Grup I: 6,4±0,564, Grup II: 3,9±0,789, p<0.001).
SONUÇ: Erkeklerde fleksibl sistoskopide %2 lidokainli jel kullanımına yarım saat önceden yapılan intramuskuler diklofenak ilavesi ile sadece üretral jele nazaran daha etkili bir analjezi sağlamaktadır.

Comparison of local anesthetic efficacy of lidocaine gel and lidocaine gel + intramuscular analgesic in patients undergoing flexible cystoscopy

OBJECTIVE: In this study, we aimed to investigate the effects of 2% lidocaine gel and 2% lidocaine gel-diclofenac combination on pain in male patients undergoing flexible cystoscopy.
MATERIALS-METHODS: Records of male patients who underwent flexible cystoscopy between 04/2016 and 04/2017 were retrospectively reviewed. Cystoscopy reasons were lower urinary system symptoms and bladder tumor. Patients with urethral stricture, additional procedures during cystoscopy (biopsy, catheter withdrawal, etc.) and patients with sensory neurological deficits were not included in the study. Patients were divided into 2 groups according to analgesia applied. 100 patients in group I were treated with 2% lidocaine gel, 109 patients in group II were treated with 2% lidocaine gel and intramuscular diclofenac. In the gel group, 10 cc of 2% lidocaine gel was instilled into the urethra 15 min before the procedure and immediately before the procedure. In the other group, intramuscular 75 mg diclofenac was applied half an hour before the 2% lidocaine gel instillation. All operations were performed by the same urologist. Post-procedure disease VAS scores were filled and groups were compared.
FINDINGS: The mean age of the patients in Group I was 63 ± 14.8 years and 61 ± 15.0 years in Group II (p = 0.42). The mean duration of the procedure was 376 ± 28 in Group I and 9 seconds in Group II (p = 0.071). There was a statistically significant difference between the groups in pain scores (group I: 6,4 ± 0,564, group II: 3,9 ± 0,789, p <0.001).
CONCLUSION: Intravenous administration of diclofenac, which is performed half a hour before the use of 2% lidocaine gel in men with flexible cystoscopy, provides more effective analgesia than urethral gel alone.
