Türkiye' de PCNL/RIRS Asistan Eğitiminde Asistanların Beklentileri ve Yeterlilikleri; ESRU Türkiye Anket Çalışması

Her iki cerrahi ile alakalı bir standardizasyon sağlanamasa da, Türkiye’ deki birçok klinikte bu eğitim verilmektedir. Çalışmamızda RIRS/PCNL konusunda Türk Üroloji asistanlarının yeterlilikleri, tercih ettikleri eğitim yöntemleri ve uygulanmakta olan eğitim modelleri oluşturulan anket ile sorgulanmış ve sunulmuştur.
MATERYAL-METOD: Anketimizde eğitimde kullanılan yöntemler, eğitimin yeterliliği ve eğitim konusundaki beklentiler odak noktası olarak belirlendi. ESRU veritabanına kayıtlı olan toplamda 295 asistana gönderilen anketler toplamda 70 (%23) asistan tarafından yanıtlandı. Anketlerin oluşturulması ve dağıtımında Survey Monkey dijital ortam kullanıldı.
BULGULAR: Anketimize en fazla katılım Marmara Bölgesin’ den (%39,29), en düşük katılım ise Güneydoğu Anadolu Bölgesi’nden (%5,36) gerçekleşti. Anketimize katılan 1., 2., 3., 4., 5. yıl asistanlarının oranı sırasıyla %14, %23, %23, %12,7 ve %25 olarak belirlendi. Eğitim araştırma hastanesi ve Üniversite Hastanesi katılım oranları sırasıyla %46,77 ve %53,23 olarak saptandı. Asistanların % 10,94’ ünün PCNL için, %14,06’sının da RIRS için kendilerini bu cerrahileri uygulamak konusunda yetersiz hissettikleri belirlendi. Asistanların vakalara katılım oranları RIRS ve PCNL için sırasıyla; primer cerrah olarak %56, %50, primer asistan olarak %25, %42,19, izleyici olarak %4, %7,81 şeklinde gözlendi. Grubun %46,88’inin her iki cerrahi için de %15,63 sadece PCNL için %15,63’ünün sadece RIRS için %21,88’inin kurslara katılmak istediği saptandı. Kurslara katılabilen asistan oranının ise RIRS ve PCNL için sırasıyla %9,34, %1,56 olduğu görüldü. Her iki konuda da kurs alabilen asistanların oranı ise %4,69 olarak belirlendi. Grubun%40,63’ünün kursa katılamaması ile alakalı “klinik içi iş yoğunluğunu” %15,63’ünün kontenjanların yetersiz oluşunu, %18,75’inin asistanların yeterince desteklenmemesini, %14,06’sının kursların bulundukları bölgeye uzak oluşunu esas problem olarak gördüğü belirlendi.
SONUÇ: PCNL ve RIRS Türkiye genelinde yaygın olarak uygulanan cerrahilerdir. Asistanların bu cerrahi prosedürler için eğitimde önemli gördüğü kursların yaygınlaştırılmasının ve asistanların bu konuda motive edilmesinin PCNL/RIRS eğitimi için yararlı olacağı düşünülmektedir.
Residency expectants on PCNL/RIRS Residency Training; ESRU Turkey Survey

Even though there is no standardization in both of these processes, this education is given by many of the clinics in Turkey. In our study qualification of the Turkish assistants about RIRS/PCNL, the chosen education techniques and education models are asked and preserved by a questionnaire.
Materials and Methods The techniques used in education, qualification of the education and expectations about the education is defined as focal point. Questionnaire was sent to total of 295 assistants who are registered to ESRU database. 70 (23%) people answered.
Results The most participation is from Marmara Region (39,29%), the least one is from Southeast Anatolian Region (5,36%). The participant assistants’ rates are 14% for 1st year, 23% for 2nd year, 23% for 3rd year, 12,7% for 4th year, 25% for 5th year assistants respectively. Training and Research Hospital and University Hospital participation rates are 46,77% and 53,23% respectively. 10,94% of the assistants see themselves inadequate for doing PCNL and 14,06% for RIRS in expert period. The rates of taking part in cases as a primary surgeon is 56%, 50%, as a primary assistant is 25%, 42,19%, as an observer is 4%, 7,81% for RIRS and PCNL respectively. 46,88% of the group wants to participate in a course for both of the processes, 15,63% for only PCNL, 21,88% for only RIRS. The rates of the assistants that could take part in courses for RIRS and PCNL are 9,34% and 1,56% respectively.The rate of assistants who could take part in both of the processes is 4,69%. 40,63% of the group show “hard work in clinic” as a reason for not taking part in a course, 18,75% show insufficient support of assistants, 14,06% show long distance between the place of the course and their home as a reason for not taking part in a course.
CONCLUSION: RIRS and PCNL are common endoscopic surgical processes in Turkey. It’s been thoght to be useful to make the courses more common which assistants see important for their education and to make assistants eager about this issue for PCNL/RIRS education.
