Secondary Complication of Inflatable Penile Prosthesis to Infection; Cavernosal Erosion

Most men with erectile dysfunction are initially offered phosphodiesterase type 5 inhibitor and then a choice of intracorporeal injection or intraurethral treatment. If these initial treatments fail or are rejected, penile prosthesis implantation is a efective treatment option. We presented a case of corpora cavernosum erosion in patient with diabetes mellitus after implantation of the inflatable penile prosthesis due to infection.
A 56-year-old patient witth diabetes mellitus presented to our outpatient clinic with complaints of the penile prosthesis with typical infection finding such as penile edema, penile pain, sensitivity, difficult micturition, dysuria and severe pain at the right side of the penis. The patient’s history revealed that the three months ago he inserted in the inflatable penile prosthesis in outside center. The patient was hospitalised and systemic treatment was given. Despite systemic treatment after four days on physical examination, erosion of the right penile prosthesis was demonstrated with a defect on the right corpora cavernosum while the left side was safe and intact. The patient was taken to the operating room and inflatable penile prosthesis was removed via a penoscrotal incision under regional anaesthesia (fig). Intra-operatively corpora were irrigated with gentamicin solution and placement of drainage tube, following intravenous systemic treatment were administered. No postoperative problems were recorded. The patient was discharged on the fourth day.

In conclusion if these initial systemic therapy insufficient, penile prosthesis implantation is a efective treatment option in men with erectile dysfunction. For all that prosthetic infection causes serious problems in terms of patient and physician. In order to protect from such as complications; risk factors should be removed and do not compromise the basic principles of treatment and penile prosthesis implantation were placement in extensive centers or implemented by surgeons with detailed training at this centers.
