One Shot Standart Perkütan Nefrolitotripsi Deneyimlerimiz ve Amplatz Renal Dilator Setin Yeniden Dizaynı

Bu set; 30F dilatator, 30F kılıf ve 8F polyurethane dilatörden oluşmaktadır. Bu çalışmada ayrıca bu setin kullanıldığı hastalarımızın sonuçlarını da sunmuş bulunmaktayız.
YÖNTEM-GEREÇLER: Şubat 2014-Temmuz 2016 tarihleri arasında one-shot PNL uyguladığımız 42 hastanın verileri geriye dönük olara incelendi. Bu teknikte sistoskopik olarak açık uçlu üreter kateteri uygulanmasını takiben prone pozisyonda iğne ile böbrek toplayıcı sisteme giriş yapıldı ve kılavuz tel ilerletildi. Kılavuz telin üzerinden 8F dilatör ilerletildi ve bunun üzerinden tek seferde 30F dilatör ile trakt genişletildi. Takiben 30F kılıf, dilatör üzerinden ilerletildi.(Resim 1)
SONUÇLAR: Hastaların yaş ortalaması 44,43±16,54 yıl idi. Ortala taş boyutu 35,12±17,53 mm idi. Ameliyatlar ortalama 54,58±22,24 dakika sürdü. Hastalar ortalama 3,09±0,75 gün hastanede tutuldu. Skopi süresi ortalaması ise 154,72±117,48 saniye idi. Tedavi başarısı 32 hastada (%76) sağlandı. Hastalarda major komplikasyona rastlanmadı. Üç hastada transfüzyon gerektiren kanama gözlendi. PNL de kullanılan tek kullanımlık dilatörlerin maliyetleri incelendi ve balon dilatör 513 TL, standart amplatz dilatör set 450 TL, ECOSET ise yaklaşık 100 TL olarak hesaplandı. Ayrıca 3 hastada trakt dilatasyonu tek seferde sağlanamadığı için "ikili dilatasyon (20F, 30F dilatörler)" (double shot) ve "üçlü dilatasyon (14F, 20F, 30F dilatörler)" (trinity shot) tekniği uygulandı. SONUÇ: ECOSET kullanılarak yapılan one-shot PNL uygulanabilir, güvenli ve etkili bir yöntemdir. Ayrıca ECOSET ticari olarak üretilebilecek olursa en ucuz tek kullanımlık dilatör set olacaktır.
One Shot Percutaneous Nephrolithotomy Experiences and Redesigning of the Amplatz Renal Dilator Set
Necmetti̇n Penbegül, Yaşar Bozkurt, Ahmet Ali Sancaktutar, Onur Dede, Mansur DaggulliPURPOSE: In this study we suggest a novel percutaneous nephrolithotomy (PNL) set that we named “Economic One-shot PNL Set” (ECOSET) which consist of a single 30F dilator, 30F sheath and 8F polyurethane dilator. Also we present our results of one-shot dilation technique in patients who underwent to (PNL) in our clinic.
Patients and METHODS: The medical records and files of 42 patients with kidney stones who had undergone one-shot percutaneous nephrolithotomy from February 2014 to June 2016 were retrospectively reviewed and analyzed. In this technique; cystoscopy was done with insertion of an open-tip ureteric catheter, followed by renal puncture in the prone position and insertion of a guidewire. The 8F polyurethane dilator was advanced over the guidewire and this was followed by directly inserting only the 30-F Amplatz dilator which carried sheath. (Figure 1)
RESULTS: By applying a “one-shot” technique, the targeted calix could be entered in 42 patients. The mean age of the patients was 44,43±16,54 years. The mean stone size was 35,12±17,53 mm. The mean operative time was 54,58±22,24 minutes. The mean fluoroscopic screening time was limited to 154,72±117,48 seconds. Treatment success was achieved in 32 patients (%76). Mean hospital stay was 3,09±0,75 days. None of the patients had any major complications during the postoperative period. Bleeding requiring blood transfusion were observed in 3 patient. The cost for a single PNL operation with a balloon set, a standart Amplatz set and a ECOSET is approximately 513TL, 450TL and 100 TL respectively.PNL tract was created in 12 patients who had previous renal surgery history but in 3 patients, the tract could not be dilated by a one-shot technique. These failed attempts were managed successfully by using amplatz renal dilators in two steps (20F, 30F dilators) that we named it double shot or in three steps (14F,20F, 30F dilators) that we named it trinity shot in the same session. CONCLUSION: Our results have demonstrated that one-shot dilation with ECOSET for PNL can be performed feasibly, safely, and effectively almost every adult patients. As a result ECOSET is the most cheapest disposable set that used during PNL surgery
