Is there a relationship between premature ejaculation treatment and plasma melatonin levels?

PURPOSE: Melatonin, secreted from the pineal gland, has been shown to play an important role in sexual function. In this study, we aimed to report the difference between the melatonin levels of premature ejaculation patients, prior to and after the treatment.

METHOD: 60 patients who applied to our clinic between January 2015 - January 2016 with lifelong premature ejaculation complaint, and 30 healthy males, as the control group, were recruited to the study. Pre-treatment IELT (intravaginal ejaculation latency time), melatonin levels, and weekly sexual intercourse frequency were recorded for each participant. 30 patients were prescribed 60 mg dapoxetin to be used before intercourse, and 30 patients were prescribed sertralin 50 mg/day. Blood melatonin levels were measured in the sixth week. All data were analyzed at the end of the study.

RESULTS: IELT and melatonin levels for Dapoxetin, Setralin and control groups are presented in Table 1. Sixth week evaluation means for all groups were as follows: In the dapoxetin group, mean IELT score was 129.3 sec. (71-183) and melatonin level was 7.52 pg/mL (2.7-13.4). For the Sertralin group, mean IELT was 126 sec. (61-187) and melatonin level was 10.6 pg/mL (5.3-18.2). Participants in the control group had a mean IELT score of 126 sec. (61-187), and their melatonin level was 10.6 pg/mL (5.3-18.2). The rise in the melatonin levels was significant for the Sertralin group, but not for the Dapoxetin group.

CONCLUSION: Melatonin levels of the premature ejaculation patients were found to be lower than healthy controls. This suggests that melatonin levels may play a role in the etiology of premature ejaculation. Our results indicate that melatonin levels rise significantly with daily SSRI-use, but not with on-demand use of dapoxetin.
