Giant Urethral Caruncle: Case Report

INTRODUCTION: Urethral caruncles are the most common benign tumors of the female urethra. They usually arise from the posterior lip of the urethral meatus and typically their sizes are not over 1 cm.
CASE: A 69-year-old woman was referred to our urology clinic with feeling of a lump in her external meatus. She had minimally sistizm complaints in her anamnesis. A 3 cm urethral caruncle was observed at external meatus on physical examination. Microscobic hematuria was observed in urine analysis. Caruncle excision was planned. A 22 Fr cystoscope was used for urethrocystoscopy. The bladder was natural. 18 Fr urethral catheter was inserted to bladder and a little traction was performed. Four 3/0 vicryl were passed through the proximally normal mucosa of caruncle without ligation of the sutures (Fig 1). Thereafter, sharp dissection was used to excise the caruncle over the sutures. Afterwards sutures were ligated and operation was finished (Fig 2). Urethral catheter was taken out and patient was discharged on the 3th day of operation. Pathological examination of the excised mass was urethral caruncle.
DISCUSSION: Urethral caruncles are accepted as benign lesions but if the macroscopic appearance is abnormal, surgically excision must be performed and pathologically malignant conditions must be ruled out. Also, excision of caruncles must be performed those reach large sizes and those may lead voiding symptoms.
