Davranış terapileri, α-adrenerjik antagonistler, topikal anestezik ajanlar,trisiklik anteidepresanlar ve selektif serotonin geri alım inhibitörleri(SSRI) tedavide kullanılmakla birlikte, erektil disfonksiyon (ED) tedavisinde kullanılan fosfodiesteraz 5 inhibitörleri de (PDE5i) PE tedavisinde denenmektedir.bir PDE5i olan tadalafil, ED tedavisi yanında günlük kullanımda benign prostat hiperplazisine bağlı semptomların giderilmesinde de kullanılmaktadır. Bu çalışmada amacımız tadalafil 5 mg’ın günlük kullanımda PE üzerine etkisini araştırmaktır. Materyal ve METHOD: Çalışmaya yaşam boyu prematür ejekülasyonu olan 30, evli erkek hasta ve gönüllü olan 30 erkek dahil edildi. Hastalara 1 ay boyunca günde bir kere tadalafil 5mg verildi.Tedavi etkinliğini değerlendirmek için ereksiyon işlevi uluslararası değerlendirme formu-5 (IIEF-5), intravajinal ejekulatuar gecikme zamanı (IELT) ve prematür ejekülasyon profili (PEP) formları tedavi öncesi ve sonrası doldurtuldu. Nitrik oksit düzeyinin değerlendirilmesi için tedavi öncesi ve sonrası venöz kan alındı. BULGULAR: hastaların yaş ortalaması 39,2±5,1 idi ve ortalama IELT 40,8±8,1s idi. Kontrol grubunun yaş ortalaması ve ortalama IELT değerleri sırasıyla 38,1±5,2 ve 196,5±26,2s olarak belirlendi (Tablo 1). Ortalama NO değerleri hasta ve kontrol grubunda sırasıyla 27,3±1,7 µmol/L ve 31,1±1,4 µmol/L olarak bulundu. Tedavi sonrasında NO değerinin arttığı görüldü.Tedavi sonrası IELT değerinin tedavi öncesi değerine göre istatistiksel olarak anlamlı şekilde arttığı görüldü (40,8±8,1s -190±19,8s) (grafik 1). PEP değerlendirmesinde tüm alanlarda istatistiksel anlamlı düzelme görüldü (grafik 2). SONUÇ: Tadalafil 5 mg günlük tek doz kullanımda prematür ejekülasyon tedavisinde etkili ve güvenilir bir ilaçtır. Effects of tadalafil 5mg once daily in the treatment of premature ejaculationLevent Özcan1, Emre Can Polat2, Alper Ötünçtemur3, Emin Özbek32Istanbul Medipol University, Faculty of Medicine, Department of Urology, Istanbul, Turkey 3Okmeydani Training and Research Hospital, Department of Urology, Istanbul, Turkey PURPOSE: Premature ejaculation is the most common sexual disorder and is estimated to affect up to 30% of men worldwide.PE can be treated with behavioural techniques, topical anaesthetics,α-adrenergic receptor antagonists,tricyclic antidepressants and selective serotonin reuptake inhibitors (SSRIs).Also, phosphodiesterase-5 inhibitors (PDE5i), which are used successfully in the management of erectile dysfunction (ED), have been tested in some clinical trials for the treatment of PE recent years.Tadalafil is a PDE5i approved in most geographies for on-demand dosing in the treatment of erectile dysfunction (ED) and once-daily dosing in the treatment of ED or signs and symptoms of benign prostatic hyperplasia (BPH), or a combination of these.In this context, we determined the relevance of serum nitric oxide levels, and we evaluated the effect of tadalafil 5mg once daily in men with premature ejaculation. MATERIAL-METHODS: Thirty married men with lifelong premature ejaculation and thirty healthy men as control group were included in this study.All the patients received tadalafil 5 mg once a day for a month.The international index of erectile function questionnaire-5 (IIEF-5) and IELT and premature ejaculation profile (PEP) were recorded before and after treatment. Plasma samples were collected before and after treatment for evaluate the level of nitric oxide. RESULTS: The mean age of the patients was 39,2±5,1 years, and the mean baseline IELT was 40,8±8,1s in the PE group; the mean age of the patients was 38,1±5,2 years, and the mean baseline IELT was 196,5±26,2s in the control group. Baseline serum NO levels were 27,3±1,7 in the PE group and in the 31,1±1,4 healthy control groups..After treatment with tadalafil 5 mg., NO levels were increased from baseline.At the end of 4 weeks, in the PE group mean IELT values showed a statistically significant improvement from the baseline values (from 40,8±8,1s to 190±19,8s ). CONCLUSION: We think that tadalafil 5 mg. once daily is safe and effective in the treatment of premature ejaculation. tablo 1
