GİRİŞ
mesane boynunun gelişimindeki anormallikler nedeniyle Konjenital retrograd ejakülasyon gelişmesi nadir bir durumdur. Bugüne kadar literatürde kollajen madde enjeksiyonu yapılan sadece 2 hasta mevcuttur. Biz bu olguda mesane boynuna submukozal kollajen madde enjeksiyonu yaptığımız ve başarı sağladığımız olguyu sunmayı amaçladık.
OLGU
28-yaşında erkek
Şikayet: dağınık idrar akımının ve boşalma olmaması
FM: RT de özellik yok,skrotal muayenede özellik yok, penis ventralinde hypospadic geniş meatus mevcut.
Semen analizi: mastürbasyonla semen mevcut değil, mastürbasyon sonrasında yapılan semen analizinde bol miktarda sperm mevcut.
Medikal tedavi:medikal tedaviye (amitriptilin ve psödoefedrin) yanıt yok
Cerrahi tedavi: sistoskopi yapıldı, veru montanum ortotopik pozisyonda normal izlendi, mesane boynu açık olduğunu(Şekil 1). Mesane boynuna Sekiz ml Dekstranomer / hyaluronik asit kopolimer (Deflux) saat 1,5,7, 11 hizasına enjekte edildi.
Dört hafta sonra ki takibinde mastürbasyonu sonrası normal 2.8 ml ejekulat mevcuttu. Sperm analizi motilitesi % 26 (a)% 57 (a + b) ve morfolojisi,% 14 ile 25 milyon sperm /ml saptandı.
SONUÇ
Önceden bildirilen konjenital retrograd ejekulasyon durumlarında yapılan kollajen maddeenjeksiyonu doğal doğurganlık için tatmin edici bir antegrad ejekulasyon sağlamaktadır. Minimal invaziv submukozal mesane boynu kollajen enjeksiyonu, hem konjenital retrogad ejekulasyonu olan hemde sekonder retrograd ejekulasyon gelişen hastalarda normal spermiogram sonuçlarına varmamızda denenmeyi hak eden bir yöntemdir.
A Successful treatment for congenital retrograde ejaculation with bladder neck collagen injection

INTRODUCTION:
Congenital Retrograde Ejaculation caused by abnormalities in the development of bladder neck is a rare condition. Only two patients were reported to have been undergone collagen injection up to today. In this report we aim to represent our case who was treated with submucosal neck bladder collagen injection and our success of treatment.
CASE:
28 year old male patient, suffering from urination problems and anejaculation. Physical examination showed normal scrotal and prostatic findings (by rectal exam). There was a megameatus ventrally located of the glans. In semen analysis, no ejaculate was able to be obtained; but postvoid urine analysis showed a lot of spermatozoa. There was no response for medical treatment (amiltriptylin and pseudoephedrine)
Surgical Treatment: Cystoscopy was performed. Veru montanum was seen to be normal in orthotopic position and bladder neck was seen to be open. 8 ml of dextranomer/ hyaluronic acid co-polimer was injected in bladder neck in 1,5.7.11 clock positions.
In 4 week follow up, 2.8 ml semen was obtained after masturbation. In the analysis motility was found to be %26 (a), %57 (a+b) and morphology was %14 normal and the count was 25 million spermatozoa per ml.
Conclusion
A satisfying antegrade ejaculation for a natural fertility is provided in the previous congenital retrograde ejaculation cases treated with collagen injection.
Minimally invasive submucosal bladder neck collagen injection is worth to try not only in patients with congenital retrograde ejaculation but also in secondary retrograde ejaculated patients to provide normal semen analysis resuts.
