Hasan Anıl Atalay, Volkan Ülker, İlter Alkan, Fatih Altunrende, Erkan Merder, Ahmet ArımanGİRİŞ: Danish Prostat Semptom Skoru (DAN-PSS-1) (Ek 1) 12 soru içeren ve alt üriner sistem obstrüksiyon (AÜSO) şikayetlerini sorgulayan bir sorgulama formudur. Bu çalışmada amacımız Bening Prostat Hiperpazili (BPH) hastalarda DAN-PSS-1 sorgulama formunun Türkçe geçerlilik ve güvenilirlik validasyonunu yapmaktır.
YÖNTEM: DAN-PSS-1 sorgulama formunun iki farklı doktor tarafından Türkçe çevirisi yapıldı daha sonra profesyonel çevirmen tarafından Türkçe çeviri İngilizceye çevrildi (Ek 2). AÜSO şikayetleri ile polikliniğimize başvuran 44 BPH hastası ile 30 kişilik kontrol grubu çalışmaya dahil edildi (Gruplar arasında yaş ortalamaları arasında anlamlı fark yoktur) (p>0,05) (Ek 3). 12 sorudan oluşan ve mesanenin depolama fonksiyonları ile işeme fonksiyonlarını sorgulayan DAN-PSS-1 sorgulama formu hastalara doldurtuldu. Her sorunun hastalara verdiği şikayet ve rahatsızlık 0’dan 3’e kadar derecelendirildi. Skor, şikayet derecesi ile rahatsızlık derecesinin çarpımı olarak hesaplandı. 2 hafta sonra hastalar çağırılıp formlar tekrar doldurtuldu. Test–tekrar test ile formun korelasyonu, kontrol grubu karşılaştırılması ile de geçerlilik ve güvenilirlik validasyonları yapıldı.
SONUÇLAR: DAN-PSS-1 sorgulama formunun iç tutarlılığının yüksek olduğu (Chronbach’s alfa:0,8), test- tekrar test korelasyonunu sonucu hastaların %98 inde aynı cevapların alındığı (Pearson katsayısı:0,98) (Ek 4) ve soruların çok iyi anlaşıldığı saptandı. İç güvenirliliği en tutarlı soru 1.soru (İşemeye başlamak için beklemeniz gerekiyor mu?) olarak bulundu (Chronbach’s alfa:0,9). DAN-PSS-1 sorgulama formunun BPH hastaları ile kontrol grubunu ayırmak için yapılan duyarlılık ve özgüllük testlerinde; kesme değer skoru 13 alındığında BPH hastalarına duyarlılığı %78,6 ve özgüllüğü ise %71 olarak hesaplandı (ROC eğrisi altında kalan alan 0,77) (Ek 5).
YORUMLAR: Bu çalışma ile BPH da AÜSO şikayetlerini ölçmeye ve değerlendirmeye yarayan DAN-PSS-1 sorgulama formunun Türkçe validasyonu yapılmıştır.
DAN-PSS-1 Questonnaire form validation study
Hasan Anıl Atalay, Volkan Ülker, İlter Alkan, Fatih Altunrende, Erkan Merder, Ahmet ArımanINTRODUCTION: Danish Prostate Symptom Score (DAN-PSS-1) is a questionnaire form which includes 12 questions and evaluates lower urinary tract symptoms (LUTS). In this study, we aimed to accomplish the validation and safety of DAN-PSS-1 in Turkish language in Benign Prostatic Hyperplasia (BPH) patients.
METHOD: Danish Prostate Symptom Score (DAN-PSS-1) form had been translated from English to Turkish by two different medical doctors and then a professional translator had retranslated to English (Table 2). Forty four BPH patients who admitted to our outpatient clinic for their LUTS and 30 controls included to study. There was no difference in mean age between the patients and controls (p>0.05) (Table 3). The patients were asked to complete the form DAN-PSS-1 which evaluates the storage and voiding functions of the urinary bladder in 12 questions. The symptoms and their severity were rated between 0 and 3 in every each question. The score was calculated as multiplying of severity rate and bothersome rate. After 2 weeks, the patients were required to come and fill the form second time. The correlation of the form was made with test-retest, the sensitivity and specificity validation were made with comparison of the control group.
RESULTS: We revealed that DAN-PSS-1 has a high self-consistency (Cronbach’s alpha: 0.8), has ability to have same answers in 98% after test-retest (Pearson parameter 0.98)(Table 4) and the questions were well understood by the patients. We also revealed that the most self-consistent question was the first question (“Do you wait for urination to start?”) (Cronbach’s alpha: 0.9). The DAN-PSS-1 system discriminated clearly between patients with benign prostatic hyperplasia (BPH) and control subjects; If the 13 cut off score taken into consideration, sensitivity of DAN-PSS-1 form was 78,6% and the specify was 71% for BPH (0.81 under ROC curve) (Table 5).
CONCLUSION: In this study a validation of Turkish of DAN-PSS-1 questionnaire form was made which used in evaluating of LUTS in BPH patients.
