Presence of PUV is the most common cause of urinary tract obstruction in the male neonates. Obstructive uropathy is the secand most common cause of chronic renal diseas and renal transplantation in children. PUV cases have rarely been detected in adults. We present a case of PUV in an adult male with history of obstructive lower urinary tract symptoms and chronic renal diseas. Material & Methods A 24-year-old male presented with obstructive lower urinary tract symptoms and cronic renal diseas. Serum creatinine level is 2.34 mg/dl at biochemical analysis and he has bilateral grade 4 hydronephrosis was detected in urinary ultrasonography. Max. Flow rate 8 ml/s, average flow rate 5 ml/ and post resudial volume (PRV) was measured in uroflowmetry. We performed cystourethroscopy and we determined PUV (Pic-1) and we performed ablation of PUV with collins knife (Pic-2) Results The bladder were determined after 1 months of operation. We found a normal flow pattern and max flow rate 17 ml/s, average flow rate 8 ml/s and PRV was detected 50 ml in urflowmetry and his serum creatinine levels were decreases to 1,68 mg/dl. Bilateral hydronephrosis was decreased. Conclusions PUV is rare codition in adults and diagnosis of PUV is also difficult. So we must consider this situation during evaluation of adult patients with obstructive symptoms especially during cystourethroscopy. In patients the long term prognosis in terms of renal and bladder function and fertility as well as the risk of malignancy in those whose bladders were augmented with gastrointestinal segments. |
