Ahmet Karakeçi, Fatih Fırdolaş, Tunç Ozan, Serkan Çarkçı, İrfan OrhanGİRİŞ: Son yılllarda DJ stentlerin Üroloji pratiğinde kullanımı artmaktadır. Bu stentlerin çıkarılması bazen unutulmakta ve vücutta taşlaşmaktadır. Burada 2 yıl önce sağ üreterorenoskopi yapılıp DJ stent takılan ve daha sonra stent çekimi yapılmayıp unutulan ve taşlaşan DJ stentin endoskopik ve peruktan yolla çıkarılma olgusu sunuldu.
OLGU: 25 Yaşında erkek hastaya yaklaşık 2 yıl önce sağ üreterorenoskopi sonrası sağ DJ stent yerleştirilmiştir. Olgu polikliniğimize sağ yan ağrısı, ateş, ve genel durum bozukluğu ile başvurdu. Yapılan tetkikler neticesinde tit: 149 lokosit, 250 eritrosit, beyaz küre: 15 bin, Hb:14.1 g/dl olarak bulundu. Çekilen direk grafide üriner sistemde sağ böbrekten başlayıp, sağ üreter boyunca devam edip mesanede sonlanan ve tamamı taşlaşmış, DJ stent saptandı. Çekilen kontrastsız Bilgisayarlı Tomografide DJ stentin sağ üreter orta kısmında 2 parçaya bölündüğü yarısının böbrek ile proksimal üreter arasında diğer parçasının distal üreter ile mesanede olduğu gözlendi. Uygun tedavi sonrası taşlaşmış DJ stentin çıkarılmasına karar verildi. ilk önce sistoskop ve holmium lazer yardımıyla mesanedeki, Üreterorenoskop ve holmium lazer yardımıylada sonra sağ üreterdeki DJ stent etrafındaki taşlardan arındırılıp dışarı çıkarıldı. En sonunda ise sağ böbrekteki kalan DJ stent parçası Perkütan Nefrolitotomi yöntemi ile etrafındaki taşlardan arındırılıp dışarı çıkarıldı.
SONUÇ: Hastalar mutlaka yapılan operasyon hakkında bilgilendirilmeli, vücuduna yerleştirilen stentin çekilmesi gerektiği vurgulanmalıdır. Tüm bu açıklamalara rağmen komplike olan ya da unutulan stentler için uygun teknik seçilmeli ve hastaların böbrek fonksiyonları korunarak stentler çıkarılmalıdır.
Removed of forgotent, fragmanted and stone Encuristated DJ Stent at the two years with Ureterorenoscopy and Percutan Guidance

Removal of these stents is sometimes forgotten and petrified in the body. In this case we present a case of removed of forgotton, fragmented and stone encuristated dj stent at two years with Ureterorenoscopy and Percutan guidance.
CASE: About 2 years ago, the right DJ stents were placed after right ureterorenoscopy to 25 years old male patient. Case admitted to our polyclinic with.right flank pain, fever, and systemic deterioration. In initial evalutaion urinalysis: 149 leukocytes, 250 erythrocytes, White blood cell count 15000, Hb:14.1 g/dl was detected. The direct graphite in the urinary tract petrified DJ stents were detected; ıt was starting from the right kidney and whether to continue along the right ureter and ended in the bladder. At the non contrast tomography ın the central part of the rigt ureter DJ stent was divided into 2 parts and. Half of the stent was obserwed between kidney and proximal ureter other parts was obserwed between distal ureter and baldder. It ıs decided to removeencuristated dj stent after appropriate treatment. Firstly help of holmium laser and Cystoscope dj stent in the bladder, then help of holmium laser and Ureterorenoscopy dj stent in the right ureter was remowed. Finally, the remaining part of the DJ stent in the right kidney was remowed with Percutaneous nephrolithotomy method purified from the surrounding stone and pulled out.
CONCLUSION: Patients must be informed about the operations, implanted stents should be noted that the withdrawal. Despite all these descriptions, forgotten or complicated stent should be remowed with selected appropriate technique.for preserving renal function of patients.
